Our new *Honeywell *HomMed Monitoring system allows our physicians to monitor your or a loved one's health seven days a week. Each day we can view; blood pressure, weight and percentage of oxygen saturation. By doing so, we identify changes in health status earlier and prevent chronic conditions from escalating into something more serious, avoiding unnecessary hospital stays and ER visits.

Call our Medicare office at 314-822-3736 and see if you or a loved one qualify for a free monitor as a Medicare homebound patient. Or, just dollars a day as a Private Support client.

With the Honeywell HomMed® Health Monitoring System, our nurses can manage your health seven days a week. In less than three minutes, health data is easily collected in your home and securely transmitted to our office. If the data collected indicates a need for intervention or if you fail to collect your health status information when prompted, we'll take appropriate action. Daily health monitoring allows us to identify changes in health status earlier and prevent chronic conditions from escalating into something more serious, avoiding unnecessary hospital stays and ER visits.

Our Medicare division has been servicing the St. Louis area and its surrounding counties for more than 10 years. Medicare services allow homebound clients who are 65+ or clients who have been disabled for at least two years to receive skilled intermittent visits in the comfort of their own home, often delaying having to receive care in a nursing home. A client must be considered "homebound" meaning that in order for the client to leave home they need a considerable amount of assistance, and when they do leave home it is not very often. We work with each person's primary physician through the entire process. If their physician orders home health, the following services may be included:


Skilled Nursing:

A registered nurse will come to your home and assess your needs, and follow a plan of care documented with your primary care physician.

Physical Therapist:
A P.T. will assist the client with therapeutic exercise for strengthening and range of motion. Also, they will help improve balance and transfer training. The family /caregiver will also receive educational training with functional activities.

Occupational Therapist:
An O.T. will assist with all the activities of daily living. They will evaluate the client's needs and recommend any adaptation equipment that may be necessary.

Speech Therapist:
A S.T. will offer hearing and speech evaluations and then make their recommendations. Their goals are to improve a client's oral and non-oral skills, reading, writing and numbers skills. They will also look at a client's cognitive skills.

Medical Social Worker:
A M.S.W. will also come into the comfort of a client's home and assist the client / family and caregivers by educating them on what community services are available and create a more comfortable atmosphere for the client.

Home Health Aide:
Our certified nursing assistant will help with the client's everyday needs. They can assist with bathing, transferring, and also light meal preparation, weight monitoring and the taking of blood pressure readings if needed. Each C.N.A. works closely with the R.N.

Companion / Home Care Aide:

$19.00 per hour / 4 hours min. (weekday and weekends)


Certified Nurses Aide (CNA):

$20.00 per hour / 4 hour min. (weekday and weekends)


Nurse Services:

$55.00 per visit


Bath Visits:

$40.00 per visit


Client Transportation / Errand Services:

* Using Client vehicle - $0.00 per mile
* Using Employee vehicle - $0.50 per mile

The managers of Algonquin Nurses, Inc. help coordinate clients and employees for a large number of cases throughout the Metropolitan Area. A great amount of effort is generated in order for clients to be serviced in a professional and expedient manner. Our clients receive care as authorized by the Division of Medical Services. Algonquin Nurses provide services for Medicaid Title 19/SSBG clients, Medicaid Waiver and Specialty Programs. This would include Aids/HIV, Mental Retardation (DMH), Healthy Child and Youth (HCY), Mideast Area Agency On Aging (MEAAA), Home and Community Based Services (HCBS) and Consumer Directed Services (CDS). We also provide services through the Home-chore Program with the Veterans Administration, however this is not Medicaid authorized.

We in the Medicaid Division of Algonquin Nurses believe and respect the rights of individuals to live independently and freely. Our Administrative staff and support staff who work directly with our clients are dedicated to serve our clients consistently in their homes and communities. Medicaid Services are supervised by a Director and Medicaid Certified Managers and are regarded as experts in the staffing field.

Algonquin Nurses will coordinate care between your Physician and Division of Medical Services or other Programs as authorized by the state. We are a member of the Missouri Council for Home Care Services and our Medicaid Division is represented by a Board Member with this organization, which assists in providing leadership in ethical business practices and highest standards of client care. Algonquin Nurses works closely with Missouri Legislators to improve the services for our clients in the community where they reside.

Our Medicaid Department staffs qualified nurse aides and nurses that service all of the St. Louis and St. Louis County area as well as St. Charles, Lincoln, Warren, Jefferson, Washington, Franklin, St. Francois, Ste. Genevieve, Gasconade, Osage, Callaway and Montgomery counties. We pride ourselves on the excellent quality of care delivered by our aides which is demonstrated by our high retention rate 80% over the last year.

 

The program includes the following services:

Homemaker Care (HC)
In-home Homemaker Care Services are general household activities provided by a trained homemaker to assist with housekeeping activities. The household tasks are necessary to maintain a safe, healthy home environment for the client.

Personal Care (PC)
In-home basic Personal Care Services are maintenance services provided in the home to assist individuals with activities of daily living (Dietary, Dressing and Grooming, Personal Hygiene and Bathing, Toileting and Continence, Mobility, Medications, and Medically Related Homemaker Tasks).

Advanced Personal Care (APC)
In-home Advanced Personal Care Services are maintenance services provided in the home (or residential care facility) to assist individuals with activities of daily living, when such assistance requires devices or procedures which are medically related to alter body functions (Bowel Programs, Range of Motion, Skin Integrity, Gastrostomy Tubes, Tracheostomy, Transfer Device, Assist with Non-Injectable Medications).

Respite Care and Advanced Respite Care
In-home Respite Care Services are maintenance services of companionship and home activities with supervision provided in the home to provide temporary relief for the regular care giver of a dependent adult. Advanced Respite Care is intended to offer relief to a care giver who lives with a client having special needs (bed fasting requiring turning, positioning, behavior disorders, prompting for self-administered medication, special monitoring due to swallowing problems such as Parkinson's, ALS and stroke).

Registered Nurse(RN) or Licensed Practical Nurse(LPN)
Authorized R.N. visits are maintenance or preventative services provided by a Registered Nurse (R.N.) or a Licensed Practical Nurse (L.P.N.)under the direction of an R.N., in the home. R.N. Visits are authorized to provide increased supervision to the Personal Care Aide, intensified health care assessment to the client, and certain nursing services which would not meet the requirements for reimbursement under Home Health Care or Private Insurance.


Healthy Children and Youth Program (HCY)

This is a comprehensive, primary and preventive health care program for Medicaid eligible children and youth until the age of 21 years. This allows children to remain in their homes as a "healthy child" or "healthy youth".

AIDS/HIV Waiver Program (HIV WAIVER)

Designed for recipients with HIV/AIDS who are currently receiving long-term care or who are at risk for institutionalization. Algonquin provides services through the St. Louis Effort for Aids, Metro Aids, Helena Hatcher Program at Washington University and Blacks Assisting Blacks with Aids. Case Managers oversee this program to insure continuity and efficiency of medical services.

Department of Mental Health (DMH)

This consists of Homemaker, Personal Care, Respite for clients who are developmentally disabled and is available to all ages.

Veterans Administration Homemaker/Chore Services/Home
Health Aide Services

Homemaker/Chore Services: Services that are necessary for the maintenance of the patient in the home. Services may include: housekeeping, laundry, meal preparation, grocery shopping, accompanying the client to appointments assuring patient safety, and other services that are consistent with the homemaker's level of responsibility and the client's care plan.

Home Health Aide Services: Services may include assistance with AD L's, aid in the ambulation of transfers, range of motion, assistance with medical equipment and routine health monitoring.

(CDS) Consumer Directed Services

HOW TO QUALIFY FOR SERVICES

For Title 19 services, call the Division of Senior Services at 314-340-7300 and ask for intake and referrals. Let the intake coordinator know you were referred by Algonquin Nurses.

For the other programs, call our Medicaid office Monday through Friday during regular business hours, 8:00 A.M. to 4:30 P.M.
